Our Process

Dr. Dan Butler is a designated Aviation Medical Examiner (AME) and can complete the necessary physical evaluation obtain your medical certificate:

  • Use MedXPress to complete the initial portion of the application.
  • Schedule an appointment with Modern Medicine by calling 337-564-1589.
  • When calling, if you have any medical conditions or take any mediation, be sure to ask the office what records you need to bring to your appointment.
  • At your scheduled appointment, we will complete your medical examination and the remainder of the FAA application form. If you meet the required medical standards, we will issue you a medical certificate.
